Palliative Care Consultancy Service
Palliative care provides coordinated medical, nursing and allied health services for people with serious health-related suffering. Our goal is to provide holistic care that is focused on maintaining or improving your quality of life. This includes not only the relief of physical symptoms, but also assistance with emotional, social and spiritual concerns.
Palliative care can be provided for people of any age and at any stage of a serious illness. The diagnosis of a life-limiting illness can be frightening. The palliative care team is here to walk the journey with you; provide support for continued quality of life; and to answer your questions. Our mission is to offer reassurance, comfort and understanding to you and your family.
What do we do?
- Pain and symptom management
- Family and carer support
- Coordinate discharge planning to other palliative care services
- Hold an outpatient symptom assessment clinic
- Assist with advance care planning and end-of-life planning
- Provide grief and bereavement support and assist with referral to other bereavement services
- Hold a biannual memorial service
