Palliative care provides coordinated medical, nursing and allied health services for people with illnesses that are not going to get better. Our aim is to provide total care; this includes the relief of physical symptoms and assistance with emotional and social concerns when facing a terminal illness.
The care we provide is for people of any age and stage of illness. We believe your wishes and choices are important; decisions should be shared whenever possible. We work closely with the primary team caring for you in hospital.
What do we do?
- Pain and symptom management.
- Hold a symptom assessment outpatient clinic Mon-Wed mornings.
- Provide guidance / support to family and carers.
- Coordinate discharge planning to community palliative care services.
- Assist with advance care planning and end of life planning.
- Provide guidance to bereavement services.
- Hold a biannual memorial service.
