Palliative Care Consultancy Service

Palliative care provides coordinated medical, nursing and allied health services for people with illnesses that are not going to get better. Our aim is to provide total care; this includes the relief of physical symptoms and assistance with emotional and social concerns when facing a terminal illness.

The care we provide is for people of any age and stage of illness. We believe your wishes and choices are important; decisions should be shared whenever possible. We work closely with the primary team caring for you in hospital.

What do we do?

• Pain and symptom management.
• Hold a symptom assessment outpatient clinic Mon-Wed mornings.
• Provide guidance / support to family and carers.
• Coordinate discharge planning to community palliative care services.
• Assist with advance care planning and end of life planning.
• Provide guidance to bereavement services.
• Hold a biannual memorial service.
