Home Link
Home Link is a nursing service based at Sir Charles Gairdner Hospital (SCGH) which provides acute outpatient care at home, or in a dedicated clinic for Sir Charles Gairdner and Osborne Park Hospital patients. Direct GP referrals are not accepted. Patients are referred by ward and ED staff to the service.
The service can access all multidisciplinary community services to help support early discharge from hospital. Services are usually provided for the short to medium term (2 to 6 weeks), and if ongoing care is required, Home Link staff will refer patients to other community service providers.
Home Link clinic is at Sir Charles Gairdner Hospital, ground floor C block.
What can I expect from Home Link care?
The nursing service will call before they visit, home visits occur between 7am to 2.30pm, and if twice daily visits are required, these will be from 4.30pm to 6pm. The nurse will do a set of observations and deliver the prescribed care. If there are any concerns during the visit, the nurse will escalate to an appropriate team within SCGH.
The service is available 7 days a week from 7am to 9.30pm on 1300 884 421, providing support for urgent enquiries afterhours.
