Home Link

Home Link is a nursing service based at Sir Charles Gairdner Hospital which provides acute care at home or in a dedicated clinic for patients from both Sir Charles Gairdner and Osborne Park Hospitals. Direct GP referrals are not accepted. Patients are referred by ward and ED staff to the service.

The service can access all multidisciplinary community services to help support early discharge from hospital.

Services are usually provided for the short to medium term (2-6 weeks) and if ongoing care is required Home Link staff will refer patients to other community service providers.

Home Link clinic is located at Sir Charles Gairdner Hospital Ground floor C block.
