Chronic kidney disease and pre-dialysis service

The Chronic Kidney Disease (CKD) and Pre-Dialysis Service provides individualised patients and carer education regarding the management of CKD and treatment options available.

• CKD management
• Dialysis treatment
  • Home dialysis therapies which include peritoneal dialysis and home haemodialysis
  • Unit and satellite haemodialysis
• Kidney supportive care

Education is delivered through a variety of modes including, clinic, telehealth and group education sessions. We offer counselling and assessment for care and planning for future care. The service works closely with Fresenius Kidney Care to provide information and assessment for all home therapies.

The service also works collaboratively with all allied health and palliative care team, with referrals into these services.
